feat: Add provider meta user-agent

Description

• Add provider meta user-agent, replacing static tag

Motivation and Context

• Attribution to our modules can now be collected through normal means by adding to the user-agent used in API requests provider: add provider_meta support hashicorp/terraform-provider-aws#45464 - we no longer need the static tags

• I have updated at least one of the examples/* to demonstrate and validate my change(s)

Actually - going to change the value to something more descriptive like author/github.com/terraform-aws-modules

Need to see what the norm is for other similar tools (if there is one)

I had the same idea after seeing it in the changelog. :)

Have you looked into how it gets merged or overridden when users wrap modules and provide their own user_agent?
